All that you have to do to register is to call in at the surgery and complete a "new patient registration" form. The registration is effective immediately. It is not necessary for all the family to come in just to register. One person can take as many forms as are needed so that their is one for each person. The forms can be completed at home and then returned to the surgery. The registration is effective as soon as we have the forms.

If you take any regular medication then it would be helpful if we could have the list of drugs on the counterfoil of your last prescription so that we can see exactly what medication you are taking. If you do not have the counterfoil then if you could let us have sight of the packs / bottles that your tablets are in so that we can note the details. If all else fails we can always phone your previous GP to get up to date details. Please give us enough time to contact your old Doctor before you run out of tablets.

 

When you register we notify West Cheshire Primary Care Trust, (PCT) and they will recall your records from your previous GP and send them on to us. Sadly it is not permissible for one GP to send records directly to another. ( This is probably far too sensible for the NHS.) It is not unusual for it to take 2-3 months for the record to finally find its way to us, so it is helpful if you can give us as much detail about yourself as possible.

When you collect a registration form you will be given a short health questionnaire to complete and return to us. This is to give us as much relevant information about you and any medical problems that you may have as possible. This information forms the start of your new record, previous details are added when we get the notes.

After you have registered you will be offered the opportunity to see one of the Nurses for a basic health check. This includes things like height, weight, blood pressure, cholesterol level, a urine test and anything else relevant at the time. You are not obliged to have this check-up however we would encourage as many people as possible to take advantage of it.

 

Who can join the practice?
